If you cannot access the web interface website (IP address), then please try the options, below:
- Reinstall CCID and EEM driver
CCID Driver: https://www.hidglobal.com/drivers/32909
EEM driver: https://www.hidglobal.com/drivers/34881
- Connect the reader to another pc (not used before) – Windows update should automatically download the driver from Microsoft
- Use HID driver (CCID) or switch to Microsoft (doc file: Driver update to Microsoft)
- Alternatively, you can reset the reader to Default settings by sending APDU command’s (the reader must be in CCID mode) in the order:
Restore Defaults ff6800010104
Store changes ff6800010103
Apply changes ff6800010101
I can also suggest checking if the reader will be visible under the Workbench application on another pc.
Additionally, I would like to add, that the Workbench Tool is a great application to test your reader installation/configuration. If the reader will be visible under the workbench then your problem is related to your current operating system of pc, laptop.

I would also suggest starting with a clean system to enforce a clean, new installation of all necessary drivers. Make sure to disconnect the reader before running the tool. Once all traces of the driver are removed, reconnect the reader. It should fix problems that are related to previous installations for this reader and ensure a fresh, clean start.
Looking at available tools to make this a little easier, You can run into the Device Cleanup tool and make some tests. This is not an HID-supported product, so no guarantee though.

Disconnect your OMNIKEY reader
Start Device Cleanup tool "As Administrator"
Select options->show device IDs
sort for device ID USB\VID_076B&PID...
->> you can limit removal to PID_5127,PID_5128, PID_5427 and PID_5428
right-click selected devices and remove
